Player / Parent Training Agreement with Soccer Goals Academy
Step 1: Please read the training agreement entirely to understand Soccer Goals Academy’s expectations. If you have any questions or concerns, text Coach Tommy at 0877627655 to schedule a phone call.

Step 2: Print out the agreement and sign the agreement stating that you have read Tommy’s terms for the training program.

Step 3: Scan the agreement and email or send a picture of the agreement to Tommy directly at [email protected]
Step 4: Once you sign and send the agreement, simply register online with the link provided in the email you received from Coach Tommy.

Step 5: We will begin working together once the steps are completed in order!

1.Terms
This Agreement will begin on (the day you register) and continue for [3 months minimum].
If you choose to cancel after 3 or 6 months, I agree to provide Coach Tommy with a 30-day prior written notice email to [email protected] to cancel any future recurring payment after the 3 or 6 month training contract is complete.
By registering for the program, you agree to the “terms and conditions” on the checkout page before you "enrol online" that clearly states Coach Tommy’s refund, cancelation details and early termination policy.

2. Player Expectations

  • I agree to be a positive player who is coachable
  • I agree to be respectful to the players and the coach's on the Academy
  • I agree to work hard in every session, I’m here to develop and reach my goals as a player
  • I agree to complete each homework tasks that Coach Tommy assigns me in a timely manner 
  • I agree to be accountable to Coach Tommy’s program and not make excuses
  • I agree to bring a full bottle of water (and proper attire) to each session, Academy strip, shin guards, gloves, correct footwear
  • I agree to work hard and achieve my personal goals that Coach Tommy and I set
  • I agree to be committed every week and dedicate time to work on my own away from our sessions
  • I agree to use the online training membership and additional resources that Coach Tommy provides for me
  • I agree that I can communicate with Coach Tommy daily about my progress on the Academy's "training app".
  • I agree to put in the work needed to become a better player
  • I agree to fill out my weekly “game evaluations” so Coach Tommy and I have a deep understanding of my performances throughout the season

    3. Parent Expectations
  • I agree to bring my child to our scheduled weekly session 10 minutes early to warm up and stretch and to be at the grounds to collect my child 5 mins before the 60 minutes is finished
  • I understand that if we are late or miss  the session, we will forfeit the time late or session missed (all sessions last 60 minutes)
  • I agree to Coach Tommy’s 48 hour only “reschedule policy” for 1 on 1 training
  • I understand that my child has 24/7 access to Coach Tommy on the Academy "training app" to ask him any questions throughout the week
  • I understand that during the sessions, I will not pressure my child or yell from the side-lines. We train in a “non-pressured” environment
  • I understand that Coach Tommy will decide the “rain-out’s” or rescheduled sessions due to weather. He will always check field conditions before the sessions and will communicate via text message or email if there is a cancellation.
  • If there is a cancellation because of weather?
  • I agree the session can be rescheduled to a Zoom meeting or Zoom training from home (9years + Group)
  • I understand to communicate with Coach Tommy (and send him via email when there are conflicts/holidays in advance) so we can reschedule sessions in a timely manner. SMALL GROUP PRIVATE TRAINING ONLY
  • I understand that if we are “out of town” or “on holidays” that we are still a member of the 3 or 6 month program and that our child has continued access to the "academy training app" to continue to work on their development anytime.
  • I understand that every 90 days Coach will be gone for 1 week (and you will receive your make-up session before he departs) or from another qualified Academy coach.

  • I understand that I will complete a minimum 16 sessions on the 1 on 1 program to be completed in 4 months.

  • I understand that Coach Tommy’s billing process is an automatic electronic funds transfer (every 30 days from the original payment)
  • I understand that if I have a “failed credit/debit card payment” I will need to register with a new credit or debit card within 48 hours of the failed payment.
  • I understand the minimum commitment of 3 or 6 months that I choose must be completed.
  • I understand Tommy’s early contract termination policy stated on his T&C no 6 on the Academy Website (https://soccer-goals-academy.mykajabi.com)
  • I understand (according to Coach Tommy’s liability no10 on T&C form) that Soccer Goals Academy, Coach Tommy and any assistant coach for Soccer Goals Academy is not liable for any injuries.
  • I agree that, to become a full member of the Academy after I Enrol my child, I will order the Academy strip on this link to confirm my Membership to avail of discounts for camps.
